Aging Life Care Management
Care Management is a professional specialty of the social service and healthcare fields. The care manager is a trained expert, generally a licensed social worker or nurse, who guides the senior and his or her family through the challenges of aging to achieve the best outcome. These challenges include a sudden illness, a chronic health condition with gradual decline in ability to manage daily tasks, memory impairment, the loss of a spouse or caregiver, a mood disorder and unanticipated financial issues.
